The Consent Form (formally named the "Authorization Form for Deposit of Student Research") is important because the Institutional Repository requires permission from students in order to host their content on the repository. This form ensures that students and their faculty advisor are explicitly aware of how the Institutional Repository will host the materials. The form requires signatures from both the student and their faculty advisor.
The form reinforces the non-exclusive license and the open access policies of the repository. Students agree to these components upon signing the form. The faculty advisor's section ensures that the work being uploaded to the repository is the one that the advisor and, if applicable, the advisory committee read and approved as the completed work. The faculty member must also agree to the repository's open access policies. This section should be completed by the primary faculty advisor.
This form must be completed for any student work uploaded in the repository. Faculty do not complete this form.
There are a few steps involved with uploading this form and the work associated with it.
First, of course, is for students and their faculty advisor to sign the form. Please fill out every field. The form is available as a fillable PDF, but a student may also print out and sign the form. If the student chooses to print the form, please scan it; the Institutional Repository will not accept any physical forms.
Second, students must navigate to the appropriate portal for uploading their work. Information for how to do this is contained later in this guide. Clicking the "Submit Research" button on the left sidebar will bring up the section for submitting.
Third, students must fill out metadata. For student capstones, theses, and dissertations, students must fill out keywords and an abstract. Please ensure you do this in order to ensure searchability of your work and to represent your work in the most accurate way possible.
Finally, students must attach the signed form as an "Additional File." One can find this at the very bottom of the submission page. After you fill out your metadata, attach your work as the main file, check this box to be able to add another file.
You will be brought to a new page that allows for you to upload additional files.

Choosing "Upload a new file" here will allow you to attach a new file. Select your filled out consent form.
Upon uploading the form, please ensure that the box below "Show" is not checked. These forms will not appear publicly; keeping "Show" checked will publish the consent form. As such, please ensure the box is not filled in. The administrators of the repository will still have access to the consent form. If there is no consent form attached, the work cannot be made publicly available. Administrators will reach out to you to obtain the form as necessary.
